Coming in for an Appointment and not well?

Here's what you need to know:

1. If you are feeling unwell please wear a mask. If you do not have a mask, there will be one at the clinic. Please continue to wear the mask for the duration of your time in the clinic. DO NOT remove it unless you are instructed to do so.

2. Please come alone if possible. If you cannot come alone, come with the minimal support required. This will help us to maintain appropriate spacing in our waiting room for other patients seeing the doctor.

Coming in for your first Appointment?

1. Please come 10-15 minutes early to your appointment as you will be needing to fill out a New Patient Registration Form.

2. Please bring your Alberta Health Care Card or out of Province Health Card.
